Abstract

Abstract Background and Objective: Nowadays, downsizing the government to have an effective and flexible organization is considered to be government’s top priority in the world and outsourcing is one of the ways to achieve this goal. Accordingly, Shahid Beheshti University of Medical Sciences has delegated some of its hospitals' duties to the private sectors. The present study has been carried out to assess the performance of outsourced clinical laboratories. Material and Methods: This Descriptive Evaluation study was carried out during 6 month (May 2011 till February 2012). The data was collected by two questionnaires with close-ended questions and one with open-ended questions, and analyzed by Wilcoxon, using SPSS software. Results: There is an improvement in outsourcing laboratory services. For example, increase in the number of tests (32%) and in the type of tests (37%). Consequently, increase of hospital income (51%). The number of personnel is decreased and their responsibility and behavior are improved. Conclusion: Overall, it seems that outsourcing laboratory program could achieve its major goals such as: Absorbing non-governmental resources in both administrative and financial aspects, omitting extra expenditure, acquiring benefits, and upgrading productivity of laboratories. Abstract

Abstract

Background and Objective: Biological weapons, like other weapons of mass destruction such as chemical, nuclear and radiological are very dangerous. In recent years, they are employed in biotterrorist attacks by many countries because of  some properties such as: the ability to make massive injury, having latent period, creating a prolonged illness, potential outbreaks and epidemics and more important because of having nonspecific symptoms and difficulty in diagnosing.

Material and Methods: The objective of this study was to describe the role of microbiology laboratories and their experts in the interventions of bio-passive defense.  In this study, we use CDC, Medline, Google Scholar, Pubmed and World Health Organization (WHO).

Result: Detection of biological agents is difficult and sometimes impossible due to features such as lack of odor, color and other physical characteristics. The most important measures for defense against biological agents are rapid detection and intervention. Thus, the laboratories should highly be equipped and the personnel be extremely sophisticated to deal with the crisis.

Conclusion: Regarding the presence of highly advanced molecular procedures, Microbiology laboratories have to be updated to deal with the potential threats .in addition,   the Laboratories professionals must be trained for the latest guidelines and specific diagnostic techniques to work with biological agents.

Abstract

ABSTRACT

        Background and Objective: Lactic acid bacteria are Gram-positive, catalase-negative, nonsporulating, either rod- or coccus-shaped bacteria that have beneficial effects on their hosts by producing antimicrobial substances such as lactic acid, hydrogen peroxide, bacteriocins and biosurfactants. Bacteriocins are antimicrobial peptides that are produced by bacteria and can inhibit the growth of other bacteria.

       Methods: In this experimental study, bacteriocin production by Lactobacilli as known probiotic strains was evaluated in different physicochemical conditions. Antagonistic activity was evaluated using quantitative method of Microscale Optical Density Assay (MODA). After neutralization of acid and treatment with various enzymes, temperature, pH and NaCl conditions, the antimicrobial activity of culture supernatant fluid of Lactobacillus acidophilus and L. plantarum was investigated against pathogenic Proteus.

       Results: The culture supernatant fluid of Lactobacilli was sensitive to proteolytic enzymes with relatively good stability to temperature. The antimicrobial activity was also present due to production of bacteriocin under different NaCl conditions (1 to 4% NaCl) and pH range of 5 to 8.

        Conclusion: It seems that the antimicrobial liquid of Lactobacillus strains contains bacteriocin, which shows antimicrobial effects against pathogenic strains of Proteus. To investigate further this effect, some complementary studies should be performed.

Abstract

Background and objectives: Turnaround time (TAT) is an important quality indicator for benchmarking laboratory performance. Delay in TAT may affect patient safety; thus, continuous monitoring and analysis of laboratory workflow is mandatory. This study was designed to improve the TAT of two biochemistry laboratories serving in tertiary care teaching hospitals (multispecialty and super-specialty) through the application of quality tools namely quality failure reporting, the Fishbone model, and process mapping.
Methods: First, TAT was defined for routine (four hours) and urgent samples (two hours). Then, TAT failureincidents in 2018-2019 were analyzed using the Fishbone model. The process map of TAT was studied and made more value streamed and lean after removal of waste steps.Corrective action plans were prioritized and implemented for potential causes with more adverse outcomes. Pilot solutions were implemented for six months and TAT failures incidents were reanalyzed.
Results: The quality failure in TAT reporting was reduced by 22% (from 34% to 12%) for urgent samples and by 19% (from 27% to 8%) for routine samples after the implementation of quality tools in multispecialty hospital laboratory. In the super-specialty hospital laboratory, the improvement was more profound and the TAT percentage achieved after the corrective actions was 96.57% and 98% for urgent and routine samples, respectively.
Conclusion: Implementation of quality failure reporting culture along with quality tools led to significant improvement in TAT and higher quality laboratory performance in terms of efficiency, reliability, and increased patient safety.

Abstract

Background and Objectives: Covid-19 is a global pandemic, caused by Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV-2). So far different clinical and hematological findings that can predict disease severity have been identified. This study explores the role of Neutrophil-to-lymphocyte ratio (NLR), Platelet-to-lymphocyte ratio (PLR) and Neutrophil-to-platelet ratio (NPR) in predicting the severity of Covid-19 infection.
Methods: After obtaining Ethics committee clearance, patients with laboratory confirmed Covid-19 infection admitted during their first two weeks of illness were included in this prospective study. NLR, PLR and NPR were derived from the CBC reports. These ratios were compared in each clinical category groups to assess the severity.
Results: The total number of cases was 160. The mean age at diagnosis was 56 years. Proportion of males were slightly higher (54.4%) than that of females (45.6%). The proportion of Category C patients (66.9%) were more than Category B (25%) and Category A (8.1%) patients. It was found that the NLR, PLR and NPR ratios has statistically significant association with severe Covid-19 infection and hence these can be used to differentiate between Category C from Category A or B. NLR is the better parameter in predicting the severity of Covid-19 disease than PLR and NPR.
Conclusions: NLR, PLR and NPR ratios can be used as predictive markers of disease severity in Covid-19 infection. Among these ratios, NLR has the highest predictive value for disease deterioration.
