Abstract

  Background and Objective : Since performance of educational services can be objectively represented by graduates’ perspective we decided to assess performance of educational services of Golestan University of medical sciences from the viewpoint of graduates.

  Materials and Methods : In­ this descriptive study­, 159 of students who graduated in the second semester of 2006-2007 academic year were asked to fill out a valid and reliable research-made questionnaire with 36 closed questions relating to­ functional factors. To analyze the data, we use descriptive and inferential statistics using SPSS software.

  Results: Of 159 subjects, 35­ (22%) are males and 124 (78%) females. Their majors are medicine, environmental health, Nursing, midwifery, Laboratory sciences, and Public health. ­The results show ­various viewpoints in different majors. ­There is significant correlation between graduates’ satisfaction with the performance of educational system and their course of study (p<0.001).­­The intense of this result is further due to medical and midwifery graduates’ dissatisfactory with the performance of high educational services and management.

  Conclusion: Based on the results, the functions in different educational sections of university are not in line with graduates’ expectations.

Abstract

  Background and Objective: Multiple sclerosis is one of the most common chronic diseases in Central Nervous system.­ its chronic nature, absence of known cure and onset in young adults results in deteriorating effect on the heath of these people. This study was carried out to determine the effect of self-management program on Health Status of patients with multiple sclerosis (­MS).

  Material and Methods: ­ the participants of this pre-experimental study were 82 MS patients referring to Iranian society of MS in Tehran selected via convenience sampling. ­The data was collected by a demographic data sheet and the health status questionnaire, and analyzed by descriptive and inferential statistics.

  Results: Before intervention, the mean of health status subscales show that there is no significant difference between two groups of case and control, While after intervention and two months later(except of pain and social function) , there are significant differences in the health subscales of two groups (p≤0.05). Furthermore, in three times of measurement , there is a meaningful difference between two groups.

  Conclusion: ­­ the results show that the self-management program ­results in improving the health status of patients with multiple sclerosis therefore, application of this supportive method could be useful to empower the MS patients and help them to manage their problems.

Abstract

Background and Objective: ‌With increasing older people population, the need to provide health care services in nursing homes is increasing. The aim of this study was to determine the effect of an educational program about the management of common problems in old age on the level of job stress in the caregivers of Golestan province nursing homes.
Material and Methods: This pre-experimental study was conducted on all 24 caregivers (11 males and 13 females) working in the nursing homes of Golestan province. The valid and reliable instruments were a researcher-made questionnaire to evaluate the knowledge and Osipow's job stress questionnaire,‌ the intervention was an educational package to increase their knowledge and skills about the management of old age problems. The questionnaires were completed before, immediately after, one month and three months after the training. The data was analyzed by analysis variance, Pearson correlation, independent t-test and paired t-test.
Results: With increasing the knowledge of the caregivers, a significant reduction in job stress scores (p<0.001) ‌was found after three months. Two components of job stress including overload (p<0.001) and insufficiency (p=0.002) showed significant decreases after three months compared to the mean scores before training.
Conclusion: Due to The effect of training on stress reduction, providing comprehensive training programs for caregivers may lead to higher quality services and health promotion for both elders and their caregivers.

Abstract

Background and Objective: Pregnancy is often introduced as a stressful experience that may be increased, and leads to some cognitive and behavioral responses called stress. By managing stress in pregnancy period, we can reduce anxiety and prevent from much of obstetric and physical complications. This study aimed to determine the effect of cognitive-behavioral based stress management on anxiety reduction of pregnant women.

Material and Methods: This pretest-posttest quasi-experimental study was conducted on pregnant women referring to health centers in Gorgan city in 2012. Twenty-four patients with eligible criteria were divided randomly into two equal groups of experimental and control. Pre-test was performed by State- trait anxiety inventory and the intervention was administered to experimental group. After that, posttest was taken for both groups. Statistical analysis was performed by covariance.

Results: Analysis of the data shows that pregnant women experience Anxiety. Stress management training in cognitive-behavioral style significantly decrease state- trait anxiety of pregnant women.

Conclusion: Stress management training in cognitive-behavioral style has an effect on state- trait anxiety reduction of pregnant women.

Abstract

Background: Time management skills are so important in education and it can be very effectiv in this process. Given that the knowledge and application of time management skills can reduce teenager's time wasting and can help them to improve their academic achievement, this study aimed to investigate the effect of time management skills on academic achievement of second grade girl students in Gorgan in the 2018 academic year. This study is experimental with pre-test and post-test with control group.
Methods: The statistical population of the present study consisted of all 2785 female students of second grade high school in 2018 academic year covered by health centers in Gorgan. Among them, 40 students were selected by random sampling method and then the samples were divided into two into treatment   and contrl group. Samples of both treatment and control groups completed the localized Britton & Tesser Time Management Questionnaire. Then, only for the treatment group, advicement were implemented. One month after the intervention, the samples (treatment and control group) completed the time management questionnaire again. At the end of the academic year, the scores of the first semester and second semester of students in the 2018 academic year were obtained. Data were finally analyzed with spss18 software.
Results: The results showed that the mean score of the control group students in the second stage decreased slightly compared to the first stage, although the paired T-test showed that this drop was not statistically significant (P> 0.05). Pre-test and post-test Wilcoxon test was used to evaluate academic achievement in the treatment group. Results showed a slight increase compared to the first step, although this increase was not statistically significant (P> 0.05). The results also showed a statistically significant difference at the level of (P> 0.05) in the mean of time management scores in the two control and intervention groups. In the subscales of short-term planning time management the two groups were evaluated before and after the intervention. Results showed a significant difference there was a statistically significant difference between the two groups in the mean score of short-term planning at (P> 0.05) level. The obtained data also showed that there was a statistically significant difference between the two groups in the mean attitude time score. The findings also indicated that there was a statistically significant difference between the two groups at the level of (P> 0.05). There is no long-term planning.
Conclusions: According to the findings, it can be concluded that the intervention did not improve the students' academic achievement, but it did improve the time management skills in the samples.

Abstract

Background: The gap between knowledge and practice is a global issue that reduces service quality by creating barriers to knowledge implementation, and thus strategies should be developed to resolve it. The present study aimed to explain barriers and strategies for implementing knowledge implementation into Iranian health system management.
Methods: This qualitative study was conducted on 53 managers and health management graduates in 2018. Participants were recruited through purposive sampling in Medical Sciences Universities in Iran. Data were collected by semi-structured interviews based on the "Promoting Action on Research Implementation (PARiHS)" framework, focus group discussions and Delphi technique. The PARiHS framework has three main elements including; evidence, context and facilitation. The Lincoln and Guba criterion was used to assess the rigor of the study. Data were analyzed in MAXQDA software version 10 using directional content analysis.
Results: After data analysis 9 subcategories and 3 main categories including; evidence, context, and facilitation were emerged. In addition to three subcategories including, culture, leadership and evaluation, the socio-political subcategory was a context for knowledge implementation main barrier in the organizational context. The systematic structural planning for the combined use of four sources of knowledge is the most important strategy.
Conclusion: The lack of context readiness in using knowledge and lack of efficient system impeded knowledge implementation in the health system management. Therefore, managers and policy makers need to design a comprehensive system based on current knowledge and successful experiences.

Abstract

Background: Time management skill learning and identification are very effective in the study process, and can reduce the adolescents' waste of time and help them increase the academic achievement. The present study aimed to determine the effect of time management skill on the academic achievement of female students.
Methods: This interventional study was a randomized controlled field trial. The statistical population consisted of all 2785 female second-grade high school students in 2017-2018 covered by health centers of Gorgan, Iran. Forty eligible students were selected and allocated into intervention and control groups. In the intervention group, a group counseling session of time management training was held for 6 hours in two days. A month after intervention, all students completed the questionnaire, and the students' grade point average (GPA) of the first and second semesters were extracted in 2017-2018. To compare the academic achievement we used the Mann-Witny U test, paired t-test, and Wilcoxon test in SPSS-16.
Results: The mean of the GPA of students in control group before and after intervention were 17.95±1.47 and 17.86±1.67, respectively (P=0.43). The GPA in the intervention group was 17.61±0.84 before intervention and showed an increase to 17.75±1.08 after the time management skill training; but the increase was not statistically significant (P=0.43). The results indicated a statistical significant difference in mean scores of 2 subscales; short-term planning and time attitude in the intervention group in comparison with the control group.
Conclusion: Despite the fact that time management skill training did not lead to the academic achievement in students, it could improve the short-term planning. Since the follow-up of the present study was short-term, it needs time to improve planning and affecting the academic achievement. It is suggested to examine this assumption in longitudinal and long-term studies.

Abstract

Background: Despite the expansion of knowledge of neonatal pain assessment and management, there is still a considerable gap between nurses' clinical practices in Iran and available evidence. The aim of this study was to develop an evidence-based protocol for neonatal pain management and to evaluate its effectiveness on nurses' clinical practice.
Methods: The present study will be based on the Johns Hopkins nursing evidence-based practice (JHNEBP) model and the three stages of practice question and project planning, evidence, and translation (PET). In this way, after forming an interprofessional team, clinical practice questions will be defined, and the main stakeholders will be identified. Then, the types, levels, and quality of evidence will be assessed to summarize their final strength using the proposed tools of the model, and finally, recommendations will be developed. In the translation phase, the recommendations will be implemented during a stepped wedge cluster randomized trial, and its implications for nursing practices in the management of pain in hospitalized neonates will be evaluated.
Results: The results of this study will lead to the production of an operational and applicable protocol in the management of pain in hospitalized neonates. It can provide the basis for improving the clinical practice of nurses and subsequently improving the quality of neonatal care.
Conclusion: One of the strengths of this study is the use of an interprofessional team approach, considering the clients' priorities, and the improvement of organizational culture in order to endeavor for knowledge translation and change in clinical practice.

Abstract

Background: Organizational knowledge management requires a structure or framework to identify, extract and document knowledge. The review of knowledge management sources shows that since the late 1980s, several cycles have been introduced in the field of knowledge management, and the Mayer and Zack cycle has been one of the most important cycles. The purpose of this study was to design a model for documenting organizational experiences based on the view of point of Iranian libraries managers in medical sciences universities by using Mayer and Zack model.
Methods: The research method was survey and correlation schemes are used. The population of the study consisted of 209 managers and heads of central and hospital libraries. Sampling was a stratified random sampling method. data gathering was done in 2022 by questionnaire. path- analyses and structural equation modeling was used to data analyses. RMS-Theta measure was used to test the model.
Results: The results of the partial least squares test showed that the gathering knowledge and Knowledge refinement has a significant effect on knowledge documentation. Knowledge refinement had the highest and knowledge application had the lowest impact on knowledge documentation.
Conclusion: The results confirmed the effects of knowledge collection and refinement on knowledge documentation. Therefore, it is possible to rely on the designed model and effective factors of knowledge documentation and generalize the obtained results for knowledge documentation in libraries and information centers.

Abstract

Background: Under ethical and legal professional obligations, nurses are required to utilize evidence-based methods to enhance the experience of children during intravenous cannulation. Non-pharmacological pain management techniques are increasingly employed in clinical settings, as they can be implemented without incurring additional costs or requiring extra time. The aim of this randomized controlled trial is to evaluate the effectiveness of a Buzzy device and distraction cards in reducing pain associated with peripheral intravenous catheter (PIVC) insertion among school-aged children.
Methods: A randomized controlled trial study was conducted on 221 school-age children patients who underwent PIVC in emergency departments. Three groups of patients were randomly assigned using a lottery method: the Buzzy device group (n=73), the distraction cards group (n=71), and the control group (n=77) receiving the standard procedure without any interventions. The children were asked to evaluate the pain level immediately following PIVC using the Wong-Baker Faces Pain Scale. Using SPSS (version 26), the independent t-test, Fisher's exact test, analysis of variance, and chi-square test were used to analyze the data.
Results: Children in the intervention groups experienced significantly less pain during the PIVC procedure compared to those in the control group, with a pain score of 5.45 ± 1.67 (p = 0.0001). There was no statistically significant difference in pain reduction between the two intervention methods: the distraction cards (2.54 ± 1.37) and the Buzzy device (2.43 ± 1.60), both showing effectiveness (p = 0.0001).
Conclusion: The results of this study indicate that both the Buzzy device and distraction cards effectively reduce pain levels during intravenous cannulation when compared to the control group. Furthermore, the two strategies demonstrated equivalent effectiveness in pain reduction, as their outcomes were nearly identical. Therefore, it is recommended that these therapeutic approaches be employed to manage and alleviate pain associated with PIVC.

Abstract

Background: Since the discovery of sickle cell disease (SCD) in the early 1990s, enormous strides have been made to reduce its global spread. More studies have focused on the management of SCD complications, with little or no information on how nurses cope with the care of hospitalized patients. This study aimed to assess the challenges nurses face in managing SCD and associated factors at Osun State University Teaching Hospital, Osogbo, Nigeria.
Methods: This cross-sectional survey study was conducted to elicit responses from 215 nurses selected through a simple random sampling technique between August and October 2022. A structured, validated questionnaire consisting of 31 items served as an instrument for data collection. Data were analyzed using SPSS. Inferential statistical analyses, including Chi-square test, independent samples t-test, and one-way ANOVA with post hoc multiple comparison tests, were conducted.
Results: Among the participants, 167 (77.7%) were female, with a mean age of 24.3 ± 0.94 years. The majority, 81 (37.7%), of the participants were aged between 31 and 40 years. The most significant challenge to SCD management was addiction to opioids and other analgesics, 174 (80.9%), while the least experienced challenge was patient signing out against medical advice 180 (83.7%). The majority of the participants, 194 (90.2%), stated that lack of financial resources to initiate treatment was the most significant factor associated with the challenges experienced in managing these patients. Significant associations were found between challenges of SCD management and the age of the nurses (P = 0.038), years of work experience (P = 0.011), and the level of professional education of the nurses (P = 0.002).
Conclusion: The administration of care for patients with SCD is hindered by multiple factors, which necessitates further investigation into the long-term impacts of educational background and job experience of nurses in the delivery of care to patients with SCD.

Abstract

Background: Inadequate pain management among critically ill nonverbal patients in ICUs is a significant public health issue that can lead to prolonged mechanical ventilation and poor quality of life. Critical care nurses play a vital role in mediating pain management, and identifying barriers they face is essential for developing effective pain relief policies. This study aimed to assess nurses' perceived barriers to pain management in ICUs.
Methods: The study employed a cross-sectional design, adhering to STROBE reporting guidelines, and was conducted from January to May 2022 across four national referral hospitals in Tanzania, including university-affiliated and public hospitals. It utilized the Eastern Cooperative Oncology Group (ECOG) barrier tool to assess critical care nurses' perceived barriers to pain management in the ICU, with data analyzed using the Kruskal-Wallis test. A total of 202 nurses were recruited through census sampling. Data collection involved demographic questionnaires alongside the ECOG barrier tool to evaluate the challenges faced by nurses in managing pain effectively.
Results: The findings revealed that there were perceived nurse barriers related to medical staff and patients with the overall mean 4.57± 1.25 and 3.90±1.17 respectively.  ICU nurses did not perceive barriers related to the healthcare system with an overall mean 2.33±0.78.
Conclusion: Critical care nurses perceive barriers related to medical staff and patients as significant obstacles to effective pain management in ICUs. Intensive training on diagnosis and assessment, along with the use of tools like the Behavioral Pain Scale (BPS) and Critical Care Pain Observation Tool (CPOT), is essential for improving outcomes. Additionally, integrating patients' cultural backgrounds into pain assessments is crucial for effective management.
