Abstract
Background and Objective: Effective communication is an essential characteristic of nursing care carried out as a necessity in education to empower patients. Therefore, this study aimed to determine Nurses' knowledge, attitude and practice about effective communication skills in Patient education.
Material and Methods: This cross-sectional study was conducted on 275 nurses selected via stratified random sampling in teaching and therapeutic centers of Esfahan University of medical sciences, 2013.We collected the data by a research-made questionnaire related to knowledge and attitude, and a checklist to observe nurses' communication skills. To analyze the data , Central indices were calculated.
Results: of 275 nurses , 86.7 % made a friendly atmosphere with their patients and 72.2% showed positive feedback, while their knowledge about communication components were 79.6 % . They (67.3%) were active listeners and they (96.7%) had close communication during patient education.
Conclusion: given that nurses' communication skills are moderate, we recommend holding communication-skills workshops especially in patient education.

Abstract
Background: Mandala Coloring and Playing are distraction strategies to reduce the anxiety level in children. However, their effects are inconsistent and have received little attention for the management and prevention of anxiety in hospitalized children. The main objective of this study was to compare the effect of distraction strategies of Mandala coloring and playing with Lego on reducing preoperative anxiety in pediatric patients undergoing surgeries.
Methods: The study was designed as a randomized, single-blind, tree-arm, parallel-group controlled trial. There were 96 children aged 6 to 18 years who were admitted to the pediatric surgery ward in Mofid Children's Hospital in Tehran-Iran. Simple randomization was used for choosing the participants for dividing them into two intervention groups and one control group. The interventions (Mandala Coloring and Playing with Lego) lasted 15-20 minutes in Mandala and Lego groups. In the control group, they received care according to the ward’s routine procedure. Anxiety as the main outcome variable was measured using the State-Trait Anxiety Inventory in two stages (before the intervention and immediately after the intervention) for children. Analysis of variance was used to compare the means in the three groups, and Tukey’s post-hoc test was used to assess the significance of differences between pairs of group means.
Results: Less state anxiety was observed after the intervention in the Lego group compared to the Mandala group (-4.69 ± 7.58, P =0.094). The data also revealed significant differences before and after the intervention in the Lego group in terms of preoperative trait anxiety (35.84 ±10.25 and 34.38 ± 8.89, respectively, P = 0.038).
Conclusion: The results demonstrated that playing with Lego and Mandala coloring, compared to the control group that received care, according to the ward routine procedure, had an effective role in reducing preoperative anxiety in children; however, playing with Lego had the most effect on decreasing anxiety state in children.

Abstract
Background: Preterm neonates are frequently exposed to painful procedures in neonatal intensive care units (NICUs), including endotracheal suctioning. Non-pharmacological interventions such as white noise may help reduce pain and improve physiological stability. This study aimed to investigate the effect of white noise on pain intensity and physiological parameters in preterm neonates during airway suctioning.
Methods: This parallel-group randomized controlled clinical trial was conducted on 50 preterm neonates admitted to the NICU of Afzalipour Hospital, Kerman, Iran. Eligible neonates were randomly assigned to intervention and control groups using a random number table. Allocation concealment was ensured using sequentially numbered, opaque, sealed envelopes prepared by an independent researcher. Neonates in the intervention group were exposed to white noise at 45 dB from 5 minutes before until 10 minutes after suctioning, whereas those in the control group received routine care. Pain was assessed using the Neonatal Infant Pain Scale (NIPS). Physiological parameters, including heart rate, oxygen saturation, respiratory rate, and body temperature, were recorded before, during, and after suctioning. Body temperature was measured using the skin temperature probe integrated into the neonatal vital signs monitor. Data were analyzed using SPSS v23. Repeated-measures analysis of variance with Greenhouse–Geisser correction was used to evaluate changes over time between groups. Independent t-test, Chi-square test, and Fisher’s exact test were applied where appropriate. A P-value <0.05 was considered statistically significant.
Results: The intervention group showed significantly lower pain scores (P=0.001), heart rate (P<0.001), and respiratory rate (P=0.010) during suctioning compared with the control group. Oxygen saturation levels were significantly higher in the intervention group during the procedure (P<0.001). No significant differences were observed in body temperature between groups (P=1.000).
Conclusion: White noise reduced pain scores and improved heart rate, respiratory rate, and oxygen saturation during airway suctioning in preterm neonates. These findings support the use of white noise as a complementary non-pharmacological intervention in NICU care.
